My dad served in the RN, guiding the Atlantic convoys. He once told me that there were many instances in which some German dockworkers who were charged with fitting the wooden shelving for ammunition would purposely use sawed off nails and other means to ensure that they were less than robust. Once out at sea, with waves tossing the ships about, the shelves would give way and the ordinance would do it's thing.

He said a few ships were lost this way; I was wondering if there are any recorded instance of this to back up my dad's assertion. Unforunately he's not here now to ask more, but if any others here have relevant info I'd really appreciate it.
