Good morning Nick

That seems to be the one document I am missing, or have been unable to find in the massive load of documents I do have. Dad also lost his Kriegie log, later in his career in a fire in the airforce offices in Dorval Quebec.
Would you have any idea how I would get the questionnaire.

As an aside, I think I mentioned previously that my Dad is still alive and his memory is still pretty sharp. We are in the process of completing a book on his wartime experience. I will likely post some of his story on the forum.

Sorry for the delay but l now have your Dad's liberation questionaire completed on 3rd May 1945. It does not tell you a lot, most of which you will already know, but it is part of the story.

I will scan in tomorrow and get it across to you.
