Civil Defense Registration form

hi all,

Does anyone know how to go about locating a chaps registration form as a civil defense fire watcher?? (presumably from TNA)

He needs documentry evidence in order to claim a Defense Medal

More likely to be with the local archives - or the local archives may have some duty logs - rotas on file which might suffice.
