Halifax V LK931 428 Squadron

Can anyone advise me of the mission and fate of
Halifax V LK931 428 Squadron lost in 1943
It was based in Middleton St.George. I looked up the Squadron and saw it was RCAF.
A crew list would also be helpful. I read that the aircraft was lost but no details other than that was available
